Common side effects include trouble sleeping, vivid dreams, headache, and nausea. Naltrexone blocks opioid receptors and must not be taken with any opioid, including prescription pain medicines and some cough or anti-diarrheal products; the combination can trigger sudden, severe opioid withdrawal, and opioid pain relievers will not work while naltrexone is active. Anyone who has been using opioids must be fully off them, typically for 7 to 10 days, before starting. Tell every clinician who treats you that you take naltrexone, since pain control in an emergency will require a different approach. Seek emergency care for severe vomiting, sweating, or confusion after any opioid exposure while taking naltrexone, which can signal precipitated withdrawal.
