This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully. This notice applies to protected health information (PHI) created or received in connection with your care.

Who this notice covers

Your medical care is provided by independent provider groups (Online Medical Care, P.C.; TMD of Kansas, P.A.; TMD of Texas, P.A.; and TMD of CA, PC), which act as the covered entities responsible for your protected health information under the Health Insurance Portability and Accountability Act (HIPAA). Velri Management LLC ("Velri") is a non-clinical company that supports your care and acts as a subcontractor business associate. Telegra MD LLC provides the technology platform as a business associate. Each of us is required by law to protect your information and to follow the terms of this notice.

How your information may be used and disclosed

Other situations where the law permits or requires disclosure without your authorization include: public health activities (such as reporting to health authorities); health oversight activities (such as audits and inspections); judicial and administrative proceedings in response to a court order or lawful process; law enforcement purposes as permitted by law; to avert a serious and imminent threat to health or safety; workers' compensation programs; coroners, medical examiners, and funeral directors; organ and tissue donation organizations; specialized government functions such as military or national security requirements; and research conducted under approved privacy protections.

Uses and disclosures requiring your written authorization. Uses and disclosures not described in this notice will be made only with your written authorization. That includes most uses and disclosures for marketing, any sale of your protected health information, and most disclosures of psychotherapy notes. You may revoke an authorization at any time by writing to us; revocation does not affect actions we already took in reliance on it.

We do not sell your protected health information. We do not use it for third-party advertising, and we do not place advertising or analytics trackers that transmit health-revealing information to third parties.

Your rights

Data location and breach notification

Your protected health information is received, maintained, stored, accessed, and transmitted only within the United States. If a breach of unsecured protected health information occurs, you will be notified as required by law.

Requests concerning care records are routed to the privacy official of the responsible provider group. You may also file a complaint with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.

Effective date and changes

This notice is effective as of June 1, 2026, and was last revised on July 29, 2026. We reserve the right to change this notice and to make the revised notice effective for information we already hold; the current notice is always posted at this page.

Who does what

Velri is a brand operated by Velri Management LLC, a Nevada limited liability company. Velri is a non-clinical technology and administrative services company. Velri does not provide medical advice, diagnosis, treatment, or pharmacy services, and does not direct the clinical judgment of any provider.

All medical consultations and prescriptions are provided directly by independent provider groups (Online Medical Care, P.C.; TMD of Kansas, P.A.; TMD of Texas, P.A.; and TMD of CA, PC), by a provider licensed in your state. Velri does not employ providers. Velri's platform runs on technology provided by Telegra MD LLC, a non-clinical technology company that connects you with the provider groups and pharmacies. A prescription is issued only if a provider determines it is clinically appropriate in their independent medical judgment.

All prescription products are dispensed by independent, licensed pharmacies. Velri does not own, operate, or control any pharmacy and does not dispense medication. You may opt out of using a given provider group or pharmacy at any time.
