When sleep stops touching your tiredness, "it's just stress" starts to feel less like an answer and more like a shrug. The good news: fatigue is one of the few complaints where a handful of ordinary blood tests can actually tell you something.
Why "just tired" deserves a real look
If you're running an office by day and managing a parent's medications by night — or back at a desk eight weeks postpartum on fragmented sleep — exhaustion has an obvious explanation. But an obvious explanation isn't always the whole story. Several common, treatable conditions produce fatigue that looks identical to burnout: an underactive thyroid, iron deficiency, low vitamin B12 or D, and disrupted cortisol rhythms. You can't tell them apart by how you feel. That's the entire point of testing.

The first tier: what a provider usually orders before anything exotic
Providers tend to start with high-yield, inexpensive tests that explain a large share of unexplained fatigue. The common opening panel looks something like this.
Thyroid: TSH first, then free T4
The thyroid sets your metabolic pace, and an underactive thyroid (hypothyroidism) causes fatigue, cold intolerance, weight changes, and mental fog. The standard first-line screen is TSH (thyroid-stimulating hormone); if it's abnormal, free T4 is typically added to clarify the picture [1]. Hypothyroidism is common and more frequent in women — U.S. survey data put the prevalence of hypothyroidism (overt plus subclinical) at roughly 4.6% of people aged 12 and older [2]. Because symptoms are vague and overlap with daily stress, thyroid disease is easy to attribute to "just life" until a lab catches it.
Iron: ferritin, not just hemoglobin
You can be low on iron before you're anemic. Ferritin reflects your iron stores and can fall while a routine blood count still looks normal, which is why fatigue from low iron gets missed. Iron deficiency is the most common nutritional deficiency worldwide and disproportionately affects menstruating and postpartum women [3]. A provider evaluating fatigue often looks at a complete blood count together with ferritin to separate "tired" from "iron-depleted."
Vitamin B12 and vitamin D
Low B12 can cause fatigue, cognitive changes, and — if prolonged — nerve symptoms; it's more common with age, certain diets, and some medications [4]. Vitamin D deficiency is widespread and also associated with tiredness and muscle complaints [5]. Both are simple to measure and simple to confirm before assuming stress is the whole answer.

The second tier: cortisol, and why it's not the first stop
"Adrenal fatigue" is a popular internet explanation, but it is not a recognized medical diagnosis — a systematic review found no scientific basis for it [6]. That doesn't mean cortisol is irrelevant; it means the real cortisol disorders are specific and tested carefully. The adrenal glands make cortisol on a daily rhythm, high in the morning and low at night. Genuine problems — too little cortisol (adrenal insufficiency) or too much (Cushing's syndrome) — are diagnosed with structured testing, often a morning cortisol or dynamic tests, interpreted by a provider [7]. This is precisely why cortisol usually comes *after* the first-tier labs: the common, treatable causes are ruled in or out first, and cortisol testing is reserved for a clinical reason rather than a hunch.
What sequencing actually looks like
Think of it as concentric circles. The inner circle is the cheap, high-yield tests that explain most fatigue. The next circle is targeted based on what the first found and on your specific symptoms. Nobody needs the entire menu on day one.
For a caregiver with zero margin, the practical value is this: one blood draw covering the first tier can either point to something fixable or clear the deck so you and a provider can talk seriously about sleep, load, and recovery instead of guessing. For someone still nursing, it also matters that a provider reviews your context — timing, medications, and what's appropriate while breastfeeding — before anything is recommended. A responsible visit asks those questions; it doesn't skip them.

Reading your own numbers without spiraling
A few grounding principles help:
- Reference ranges are population bands, not personal targets. A result inside the range can still warrant a conversation if your symptoms are loud.
- Trends beat single points. One ferritin or one TSH is a snapshot; how it moves over time is often more informative.
- Context changes interpretation. Recent illness, pregnancy and postpartum, menstrual timing, and some supplements all shift results. That's why a provider — not an app — reads them.
Seeing your own labs is reasonable and empowering. Interpreting them in isolation is where people get lost, because the same number means different things in different bodies.
Questions worth bringing to a visit
- "Given my symptoms, does a first-tier fatigue panel — thyroid, iron/ferritin, B12, vitamin D, and a basic blood count — make sense for me?"
- "If those are normal, what would justify checking cortisol?"
- "Is anything you'd suggest appropriate for my current situation — including if I'm breastfeeding?"
- "Can I see my results and understand where my baseline sits?"
None of these require chasing optimization. They're the questions that separate a treatable cause from a season you have to navigate differently.
