Why a single lab snapshot misleads

As a clinician you already know the trap: one estradiol draw on one morning tells you almost nothing about a perimenopausal woman whose hormones swing week to week. The Study of Women's Health Across the Nation (SWAN) documented that the menopause transition is characterized by highly variable, non-linear hormonal patterns rather than a clean downslope [1]. That variability is exactly why function-and-composition metrics, trended over time, often tell a more honest story than any one-time hormone value.

Two of the most durable of those metrics are grip strength and appendicular lean mass. Both are cheap or moderately priced to measure, both are reproducible, and both have been studied as markers that track with broader health rather than vanity.

Grip strength: a small measurement with outsized signal

Grip strength is measured with a hand dynamometer in a standardized position, and it functions as a proxy for total-body strength and neuromuscular integrity. Appendicular lean mass and what DEXA actually tells you

Appendicular lean mass (ALM) is the lean soft tissue in your arms and legs, most precisely quantified by dual-energy X-ray absorptiometry (DEXA). Clinicians commonly normalize it to height (ALM divided by height squared) to create an index used in screening for low muscle mass. The hormonal and recovery picture, measured honestly

Longer recovery and fragmented sleep are not automatically "in your head," and they are not automatically hormonal either. From metrics to a strength-preservation plan

Once you have baselines, the levers are unglamorous and well supported.
