# Persistent fatigue: is it workload, poor sleep or something else?

Ongoing fatigue can have several causes. A useful evaluation connects the symptom pattern with sleep, health history, and selected tests.

A demanding schedule can leave someone exhausted. It can also make another cause of fatigue easy to overlook. Persistent fatigue deserves more than a quick label of burnout, especially when rest no longer brings the usual recovery.

Fatigue has many possible contributors, including disrupted sleep, anemia or low iron, thyroid conditions, medications, depression, and ongoing pain.[1] More than one can be present.

## Describe the change, not just the workload

The most useful starting point is what has changed. Feeling sleepy during quiet moments is different from losing strength, becoming breathless with familiar tasks, or feeling mentally drained.

Timing helps too: when symptoms began, whether they followed an illness, and whether days off make a difference. New medicines, changes in eating, and changes in mood are relevant even when the main complaint is physical exhaustion.

These details help decide what needs examination or testing. There is no single blood test that separates work stress from every medical cause of fatigue.

## Sleep quality can be easy to miss

Time in bed does not always equal restorative sleep. Loud snoring, gasping during sleep, or marked daytime sleepiness can suggest a sleep problem worth evaluating. Sleep apnea involves repeated interruptions in breathing and may require a sleep study to diagnose.[2]

A normal blood panel would not settle that question.

## Testing should follow a reason

Depending on the history, an evaluation may include tests for anemia, thyroid function, glucose, or other possible contributors.[1] Ordering a larger panel is not automatically a more thorough answer.

Likewise, a result within the reference range does not explain away symptoms that continue. Follow-up can revisit the history, check whether the original explanation still fits, and decide whether another type of assessment is needed.

A short note about sleep, symptoms, and the tasks that have become harder can be more useful at that visit than trying to recall a month of exhaustion from memory.
