# Tadalafil and blood-pressure medicines: the interaction review

Tadalafil can add to blood-pressure lowering. Nitrates are contraindicated; other combinations require a medication-specific review.

Tadalafil can lower blood pressure and can add to the effect of some other medicines. Nitrates are a specific contraindication, not simply a combination that needs a smaller dose. Other blood-pressure medicines require a review of the exact drugs and clinical situation. [1]

## Start with a complete medication list

The review should include prescribed medicines, over-the-counter products, supplements and drugs used only occasionally. A list that says “blood-pressure pill” does not identify the interaction.

The tadalafil label warns about use with nitrates and guanylate cyclase stimulators. It also describes the potential for added blood-pressure lowering with alpha blockers and other antihypertensive medicines. These categories are not interchangeable. [1]

## Why symptoms and timing matter

Added blood-pressure lowering may cause dizziness or fainting. The label notes that fluid depletion and other antihypertensive medicines can affect the safety of some combinations. Alcohol can also contribute to blood-pressure lowering. [1]

The prescribing provider needs to know about fainting, low-pressure symptoms, heart history and recent changes in the medication list. A home blood-pressure number alone does not clear an interaction.

## Chest pain changes the situation

Someone who develops chest pain after taking tadalafil should seek urgent medical help and tell the responding team when it was taken. The label warns against self-administering nitrates and makes nitrate decisions in a life-threatening situation a matter for monitored medical care. [1]

An online timing rule should not be used to decide independently when a contraindicated medicine becomes safe.

## Tadalafil is not a general libido treatment

Its effect on erectile function should not be confused with an explanation for reduced desire. Desire and erections are different parts of sexual function, and persistent symptoms may have other causes. [2]
