# A semaglutide plateau: what to review before switching

A pause in weight change is a reason to review the treatment, not an automatic reason to switch to tirzepatide.

A period without further weight loss does not automatically mean semaglutide has stopped helping or that tirzepatide is the next step. Weight-management treatment is reviewed for benefit, tolerability and the person's broader health, not one scale reading. [1]

## Establish what has actually changed

A review is more useful with a dated trend than a remembered lowest weight. Treatment duration, prescribed changes, missed doses and interruptions in supply help describe the course of treatment.

The goal is an accurate account, not a test of willpower. A medication plan that has been difficult to follow needs that difficulty identified before its effect can be interpreted.

## Include symptoms and daily function

Appetite changes, gastrointestinal symptoms and ability to maintain food and fluid intake matter. Semaglutide's label describes nausea, vomiting, diarrhea and other adverse effects, including dehydration-related kidney injury. [2]

A lower number on the scale is not the only relevant finding if treatment is also causing persistent symptoms. Conversely, stable weight after earlier loss may need a different discussion from no meaningful benefit since starting.

## A switch needs a specific reason

NIDDK describes several possible responses when a weight-management plan is not working as intended, including reviewing lifestyle support, other medicines and alternative treatment. Switching is one option, not a conclusion produced by the word “plateau.” [1]

Tirzepatide has its own contraindications, adverse effects and reproductive considerations. Its dual-receptor mechanism does not eliminate those questions. [3]

## Avoid an improvised transition

The prescribing provider should determine whether to continue, change or stop treatment and give product-specific instructions. The labels caution against combining these products with other GLP-1 medicines. [2][3]

The review should leave a clear reason for the next step and a date to reassess its effects.
