# Semaglutide and a higher resting heart rate

Resting heart rate can rise during semaglutide treatment. A wearable trend is useful context, but symptoms and clinical assessment matter.

Semaglutide's approved labeling reports increases in resting heart rate. A new upward trend on a watch is therefore relevant to the prescribing provider, but it does not show the cause or significance by itself. [1]

The label calls for heart-rate monitoring and for patients to report palpitations or a racing heartbeat while resting. For a sustained increase, the label instructs clinicians to discontinue treatment. The prescribing provider should assess the change and direct the response. [1]

## Compare similar readings

A watch reading during a walk is not comparable with a resting reading. A useful record identifies the device, time, activity and whether symptoms were present. Keeping that context helps prevent an isolated number from becoming a misleading trend.

A wearable can bring a concern to attention. The label calls for clinical monitoring of heart rate, including assessment of a sustained increase. [1]

## Report the experience as well as the number

“Racing while sitting still” and “my weekly average moved slightly” are different reports. Include when the change began, whether it followed a treatment change and whether there has been vomiting, diarrhea or difficulty maintaining fluids. Dehydration is a separate concern in semaglutide's safety labeling. [1]

A provider may need to check the pulse, review other medicines and assess possible contributors before deciding what the trend means.

## Do not build a self-adjustment rule from trial averages

The average change reported in a trial is not a personal safety cutoff. It does not mean that every smaller change can be ignored or that every larger change proves harm.

The useful response is to review a persistent resting change and any associated symptoms, following the medication's safety instructions. A dose should not be adjusted independently to make a watch score return to its previous value. [1]
