# Night cramps on semaglutide: more than a magnesium question

Night cramps have several possible causes. Fluid loss, activity and medication history matter before assuming a mineral deficiency.

Night cramps do not establish a magnesium deficiency. Muscle cramps can follow overuse, fluid loss, electrolyte changes, nerve problems or medicines, and sometimes no clear cause is found. [1]

Semaglutide adds a relevant question: has nausea, vomiting or diarrhea changed the person's ability to eat and drink? Its label warns about dehydration and associated kidney injury. That deserves attention without assuming every cramp comes from the medication. [2]

## Start with the pattern

The location, duration and frequency help describe what is happening. A brief calf cramp after an unusually active day is different from persistent weakness or repeated episodes that disrupt sleep. Report any swelling, redness or warmth as well. [1]

Changes in activity, recent illness and other medications belong in the same review. Focusing on one supplement can miss those details.

## What a blood test can and cannot settle

Electrolytes such as magnesium, potassium and calcium contribute to muscle function. Low levels are among the possible causes of cramps, but symptoms alone do not identify which mineral, if any, is involved. [1]

A provider may select tests based on the history. The question is whether testing can clarify a suspected problem, rather than whether every cramp should trigger a large mineral panel.

## When symptoms need attention

MedlinePlus recommends medical review for cramps that are severe, frequent, prolonged or accompanied by weakness, swelling, redness or warmth. Persistent vomiting, diarrhea or difficulty maintaining fluids during semaglutide treatment also needs timely assessment. [1][2]

Magnesium supplements are not risk-free. They can cause diarrhea and interact with medicines, and impaired kidney function increases the risk of magnesium toxicity. [3]

The clearest next step is an explanation of the cramp pattern and any fluid loss. Adding magnesium before that explanation may create a new problem without resolving the original one.
