# Caregiver fatigue: what thyroid and iron tests can tell you

Persistent caregiver fatigue deserves context. Thyroid and iron tests answer specific questions; cortisol does not measure the burden of caregiving.

Thyroid and iron tests can help investigate fatigue when symptoms or history point toward those causes. They cannot measure the total burden of caregiving, and a normal result does not make exhaustion imaginary. [1][2][3]

An evaluation starts with the pattern: when fatigue began, how sleep has changed, whether it improves with rest and whether there are other symptoms. Medicines, mood, pain and daily demands can all be relevant. [1]

## Thyroid tests assess thyroid function

TSH is the signal from the pituitary to the thyroid. Free T4 may help interpret it. These results can clarify whether thyroid function is part of the problem, but they do not function as a general score for energy or resilience. [2]

Different thyroid tests have different purposes. Ordering more of them is useful only when the additional result addresses a question that remains unresolved.

## Ferritin helps assess stored iron

Ferritin is an iron-storage protein. Its blood level can help assess iron stores, often alongside other tests. Both too little and too much iron can be harmful, so the result should guide interpretation rather than trigger an automatic supplement purchase. [3]

The reason for low iron, when present, matters as well as the value. The test is one part of an investigation, not a complete explanation of why someone is tired.

## Cortisol is not a caregiver stress meter

The Endocrine Society states that “adrenal fatigue” is not a scientifically established condition and that tests marketed for it are not validated. That does not mean fatigue is unimportant; it means that the label can distract from an actual cause. [4]

Caregiving strain and a medical condition can coexist. The assessment can acknowledge both without attributing every symptom to stress.
