# Postpartum shedding: when thyroid and iron tests may help

A scalp exam and symptom history help determine whether postpartum shedding needs thyroid, iron or other testing.

Blood tests can help explain postpartum hair shedding when the history or scalp examination suggests another cause. They are not a routine requirement for every person who sheds more hair after birth. [1]

Timing is the starting point. Typical postpartum shedding appears after delivery and often peaks around month four. Most people recover their usual fullness within a year. A pattern that does not fit, or symptoms beyond hair changes, can make a closer evaluation useful. [2]

## What the examination adds

A dermatologist asks when the shedding began and whether it came on suddenly, looks at the scalp and checks the hair itself. This helps distinguish shedding from breakage or patterned thinning. More than one cause can be present. [1]

That distinction determines the value of testing. A blood test does not replace an examination, and a result inside the laboratory range does not identify the type of hair loss.

## When thyroid testing enters the picture

Postpartum thyroiditis is inflammation of the thyroid after childbirth. It can involve a period of excess thyroid hormone, a period of low thyroid hormone, or only one of those phases. Symptoms can overlap with the demands of caring for a baby: fatigue, sleep difficulty and changes in weight or mood. [3]

TSH is commonly the first thyroid blood test. Free T4 may help interpret an abnormal TSH or clarify the clinical picture. Thyroid antibodies answer a different question about possible autoimmune disease. [4]

## Iron is a question, not an automatic supplement

Ferritin can help assess stored iron. A low result can support iron deficiency, while inflammation and other conditions can raise ferritin. The number is interpreted with the history and other findings. [5]

The Academy's hair-loss guidance describes testing for suspected deficiencies when appropriate and cautions against supplements when they are not needed. [1]

Testing works best when it follows the pattern of shedding and any accompanying symptoms. A large panel cannot explain a postpartum change simply by producing more numbers.
