# Postpartum fatigue: what ferritin and cortisol can tell you

Iron status can matter after childbirth, while cortisol tests answer specific endocrine questions. Persistent fatigue needs its full context.

Ferritin and cortisol answer different questions after childbirth. Ferritin helps assess iron stores. Cortisol testing is used to investigate specific disorders of cortisol production; it is not a general measure of how depleted someone feels. [1][2]

Persistent postpartum fatigue deserves an assessment that includes the delivery, recovery, sleep, mood and accompanying symptoms. [3]

## Where iron testing can help

Blood loss during childbirth can contribute to iron-deficiency anemia. The assessment may include a complete blood count and iron-related tests, including ferritin, depending on the history and findings. [4]

Low ferritin can indicate depleted iron stores. A higher ferritin result is more complicated because inflammation and other conditions can raise it. The result is interpreted with the rest of the assessment. [1]

## Why cortisol is a different test

Cortisol normally changes through the day. Stress, exercise, medicines and other circumstances can affect a measurement. Clinicians use cortisol tests when investigating conditions such as Cushing syndrome or adrenal insufficiency, with the timing and test type chosen for that purpose. [2]

An isolated value does not identify the cause of fatigue.

## Thyroid changes can be part of the picture

Postpartum thyroiditis may cause a period of excess thyroid hormone followed by an underactive phase, although not everyone experiences both. Symptoms can include fatigue, palpitations or anxiety early on, and fatigue, constipation or dry skin later. The timing over the months after delivery helps guide evaluation. [5]

Urgent symptoms need prompt care rather than a routine lab appointment. CDC lists chest pain, trouble breathing, fainting and sudden overwhelming weakness or tiredness among maternal warning signs during pregnancy and the year after delivery. [6]
