# Visceral fat around menopause: why the scale can miss a change

Research finds changes in abdominal fat and lean mass during menopause. What those findings mean, and what a waist measurement cannot prove.

Body composition can change during the menopause transition without an equally striking change on the scale. Research has found increases in fat mass and declines in lean mass around this period, changes that can partly offset one another in total body weight. [1]

## What studies have observed

In the longitudinal SWAN study, the rate of fat gain accelerated and lean mass began to decline during the transition. Total weight did not show the same acceleration. The study followed changes over time rather than testing a treatment. [1]

A separate SWAN Heart analysis followed 362 women and examined visceral adipose tissue, the fat within the abdomen. Visceral fat accumulation accelerated in the years around the final menstrual period. During part of the transition, greater visceral fat was associated with a measure of thickening in the internal carotid artery wall. [2]

That was an association in an observational study. It does not predict one person’s cardiovascular outcome or prove that menopause alone caused the change. [2]

## What visible changes can and cannot tell you

A change in clothing fit can make this shift noticeable. It cannot quantify visceral fat or establish the reason for a metabolic change. The research used body-composition measurements, not appearance alone. [1][2]

Metabolic assessment answers a separate question. NIDDK identifies abdominal body fat as one risk factor for insulin resistance, but prediabetes is assessed with glucose-based tests such as A1c, fasting glucose or an oral glucose tolerance test. Insulin resistance itself often has no symptoms. [3]

The useful interpretation combines what has changed in body composition with actual health measurements.
