# Perimenopause and GLP-1 medicines: clarify the weight and glucose questions

How changing body composition, glucose results and medication eligibility fit together during perimenopause.

Perimenopause can coincide with changes in body composition, but that does not make every weight change a sign of insulin resistance. A GLP-1 discussion is clearer when it separates the weight-management goal, glucose status and menopause symptoms. [1][2]

## What may be changing

In the longitudinal SWAN study, fat gain accelerated and lean mass declined during the menopause transition. The rate of total weight gain did not accelerate in the same way. This was an observational study of a group, not a forecast of one person’s weight trajectory. [1]

Those findings help explain why body changes can feel different even when the scale has not moved dramatically.

## What glucose testing can establish

Insulin resistance means the body’s cells respond less effectively to insulin. It often has no obvious symptoms. Prediabetes is assessed using tests such as A1c, fasting plasma glucose or an oral glucose tolerance test; tests that directly measure insulin resistance are mainly used in research. [2]

## Where medication selection fits

Weight-management medication eligibility considers body size and weight-related health conditions. The choice also depends on medical history, other medicines, potential adverse effects, preferences and cost. NIDDK emphasizes that response varies and that medication is used with nutrition and activity support. [3]

Menstrual and sleep symptoms still deserve their own assessment. Hormones fluctuate through perimenopause, and pregnancy can remain possible even with irregular periods. [4] Weight-management medicines should not be used during pregnancy or when planning pregnancy, making reproductive plans relevant to the discussion. [3]

A coherent plan identifies what each part addresses: symptoms of the transition, a measured glucose problem, or a weight-related health goal.
