# Hair thinning during perimenopause: what else can explain it?

Hair thinning in midlife may have more than one cause. How scalp findings, iron status, thyroid symptoms and timing guide the assessment.

Hair thinning that starts during perimenopause still needs a hair-loss assessment. The timing is useful, but it does not tell you whether the problem is increased shedding, hereditary thinning or another condition. More than one cause can be present. [1]

## The scalp and timeline come first

A dermatologist considers the history and examines the hair and scalp before deciding which tests are useful. Blood tests or a scalp biopsy may be appropriate when the findings suggest an underlying disease, deficiency, hormonal issue or infection. [1]

Recent illness, major stress and substantial weight loss can precede increased shedding by several months. That delay can make a current hormonal change seem like the only explanation when the relevant trigger happened earlier. [2]

## Iron testing answers a specific question

Ferritin reflects stored iron. A low result can support iron deficiency, but a high result has several possible explanations, including inflammation. It is interpreted with the blood count, symptoms and other findings. [3]

Heavy menstrual bleeding is one recognized cause of iron-deficiency anemia. A change in bleeding pattern therefore matters to the history, especially when thinning accompanies fatigue or other symptoms. [4]

## Thyroid symptoms can change the workup

An underactive thyroid can cause fatigue, dry or thinning hair, cold intolerance and menstrual changes. These symptoms overlap with other conditions, so thyroid disease cannot be diagnosed from the hair change alone. [5]

The aim is to connect a finding with a plausible cause: a scalp pattern, a shedding timeline or evidence of an underlying condition. When several causes coexist, the explanation may need to account for each alongside the others. [1]
