# GLP-1 Maintenance and “Microdosing”: What the Labels Establish

Maintaining weight loss is a treatment goal. “Microdosing” does not identify an FDA-approved regimen or guarantee fewer side effects.

Maintaining weight loss is an established treatment goal for approved semaglutide and tirzepatide products. “Microdosing” is not an FDA-approved regimen described in their prescribing information.[1][2]

The term does not specify a standardized amount, schedule, or evidence-based maintenance plan.

## Starting and maintenance doses have different roles

Approved prescribing information distinguishes treatment initiation, dose escalation, and maintenance. The appropriate plan considers the product, indication, response, and tolerability.[1][2]

A small starting amount should not automatically be interpreted as an effective long-term maintenance regimen. A social-media schedule also cannot establish how someone should change an existing prescription.

## Maintaining a result still requires review

A plateau after weight loss may represent maintenance rather than loss of all treatment benefit. The review considers the full treatment course, appetite, side effects, nutrition, and relevant health conditions.

Weight-management medication may be used long term when beneficial and appropriate. NIDDK notes that weight regain can occur after stopping treatment.[3] That does not predict every individual's experience, but it makes an ongoing plan important.

## Lower does not automatically mean safer

Side effects and suitability depend on more than a dose label. Approved products have specific warnings, contraindications, and instructions.[1][2]

Compounded preparations add a separate issue: they are not FDA-approved, and FDA does not review their safety, effectiveness, or quality before marketing.[4] Calling a preparation a microdose does not remove that distinction.

## Make the proposal specific

A maintenance discussion should identify the exact medication and preparation, the intended goal, what will be monitored, and when the plan will be reassessed.

If a service uses the word microdosing, the prescriber needs to explain the actual proposed regimen and the evidence behind it.
