Vitamin B12 deficiency can cause fatigue, but it is one possible explanation. Food intake, symptoms and risk factors guide testing.

Vitamin B12 deficiency can cause fatigue and sometimes neurological symptoms such as tingling or numbness. It is one possible explanation for tiredness, not a diagnosis that follows automatically from starting a GLP-1 medication. [1]

A useful assessment considers symptoms, food intake and factors that affect B12 absorption, alongside other possible causes of fatigue. [1][3]

## Appetite changes make intake relevant

When appetite falls, people may eat less or avoid foods they previously ate regularly. A joint nutrition advisory for GLP-1 treatment recommends reviewing dietary intake and the possibility of nutrient deficiencies. [2]

Mention a restricted diet, difficulty tolerating food and any supplements already being taken. B12 status can also be affected by absorption problems and certain medicines. [1]

## Testing may need more than one result

Serum B12 is commonly used to assess status. When the result needs clarification, methylmalonic acid, or MMA, may help. Kidney function can affect MMA, so it also needs interpretation in context. [1]

B12-related neurological symptoms can occur without anemia. That is a reason to describe symptoms clearly rather than relying on a normal blood count to settle the question. [1]

## Keep the broader fatigue picture in view

Poor sleep, anemia, thyroid problems, medicines and other conditions can contribute to fatigue. The history and examination help determine which tests are relevant. [3]

For a treatment review, a concise account is useful: when tiredness began, whether it is getting worse, how it affects daily tasks and whether eating or sleep changed at the same time.
