# TSH and the GLP-1 thyroid warning answer different questions

TSH assesses thyroid function. It does not replace the personal and family history review required by semaglutide and tirzepatide warnings.

A TSH blood test assesses thyroid function. The boxed warnings for approved semaglutide and tirzepatide products concern thyroid C-cell tumors, including medullary thyroid carcinoma, or MTC. A normal TSH result does not replace the medical-history review those warnings require. [1][2][3]

## What TSH tells a provider

Thyroid-stimulating hormone helps regulate the thyroid's hormone production. Testing can help identify an underactive or overactive thyroid, and additional tests may be needed to understand an abnormal result. [1]

Symptoms such as fatigue or weight change may lead a provider to consider thyroid testing. They do not make TSH a test of whether a weight-management medication is suitable.

## What the boxed warning says

The approved semaglutide and tirzepatide labels describe thyroid C-cell tumors in rodents. Whether these medications cause such tumors in humans is unknown. Both products are contraindicated in people with a personal or family history of MTC or with multiple endocrine neoplasia syndrome type 2, known as MEN 2. [2][3]

That history needs to be discussed directly. If a relative had thyroid cancer and the type is unclear, explain that uncertainty rather than assuming it is or is not relevant.

## Bring the diagnosis, if available

Useful information includes the name of a past thyroid diagnosis, relevant reports, current medicines and any known family history. “Thyroid disease” is a broad description; the specific condition helps the provider interpret it.

The semaglutide label also advises reporting a neck lump, persistent hoarseness, swallowing difficulty or breathing difficulty. It states that routine calcitonin testing or thyroid ultrasound has uncertain value for early detection of MTC during treatment. [2]
