# Sleep changes after starting a GLP-1: what to pay attention to

Digestive symptoms, fatigue and an existing sleep problem can overlap. Describe the change in sleep rather than assuming one cause.

If sleep changes after starting a GLP-1 medication, describe what changed: trouble falling asleep, waking with discomfort, repeated awakenings or unusual daytime sleepiness. The timing is useful information, but several possible contributors may overlap.

The approved semaglutide label lists fatigue and digestive symptoms, including nausea and reflux. These deserve attention when they interfere with daily life or rest. [1]

## Separate poor sleep from feeling tired

Being unable to sleep and feeling tired despite sleep are different experiences. A person waking with heartburn has a different starting point for assessment from someone whose partner notices loud snoring and pauses in breathing.

NHLBI lists frequent loud snoring, gasping and breathing that starts and stops among sleep-apnea symptoms. Daytime tiredness, fatigue and difficulty concentrating can also occur. A sleep study may be needed to establish the diagnosis. [2]

Mention symptoms that existed before treatment as well as new ones.

## Bring a short timeline

A sleep evaluation may use a diary of sleep and wake times, alongside information about daily routines, caffeine and other relevant habits. [3]

For a medication-related discussion, useful notes include when treatment began, any prescribed changes, when sleep became different and whether nausea, vomiting or reflux occurs at the same time. Parents and shift workers can also describe unavoidable interruptions to sleep.

## Persistent symptoms need review

The semaglutide label advises contacting the prescriber about severe or ongoing stomach symptoms. Repeated vomiting or diarrhea can also cause dehydration and should be reported promptly. [1]

If pregnancy or breastfeeding is relevant, raise it directly with the prescriber; medication-specific guidance is part of that assessment. [1]
