# GLP-1 “microdosing” for maintenance: what is established?

Maintenance trials support continued treatment in specific regimens. They do not establish that any tiny dose or intermittent schedule will preserve results.

The term “microdosing” is not enough to identify a maintenance plan. It may be used to describe a smaller amount, a longer interval between doses or another departure from a standard regimen. The actual product and prescribed schedule are what can be evaluated.

Approved semaglutide and tirzepatide labels define specific maintenance regimens and direct clinicians to consider treatment response and tolerability. They do not define a separate “microdosing” category. [1][2]

## What do maintenance trials support?

SURMOUNT-4 compared continuing tirzepatide with switching to placebo after an initial treatment period. Continued treatment maintained and added to the initial weight reduction, while withdrawal led to substantial regain on average. [3]

The trial did not compare continued treatment with smaller doses or longer intervals between doses, so it cannot establish how those approaches would perform. [3]

The semaglutide STEP 1 extension likewise followed people after treatment and the lifestyle intervention stopped. It found substantial average regain, but it did not test microdosing as an alternative. [4]

## What would make a proposed plan reviewable?

A meaningful explanation should identify the ingredient, formulation, intended regimen and evidence being used. It should also say how benefit and tolerability will be assessed and what would prompt a reassessment. [1][2]

## Why compounded products add another distinction

Compounded drugs are not FDA approved, and the FDA does not review their safety, effectiveness or quality before marketing. Evidence for an approved product should not be treated as proof of equivalent performance from a different compounded preparation. [5]

The FDA has also reported dosing errors involving compounded injectable semaglutide, including confusion about concentrations and syringe units. A smaller intended amount still requires exact, product-specific instructions. [6]
