# GLP-1 maintenance: what happens after reaching a goal?

Withdrawal trials show that weight regain is common after treatment stops. They inform maintenance planning but do not predict one person’s future.

Reaching a weight goal does not automatically end the need for treatment. Studies of semaglutide and tirzepatide show that weight regain is common after withdrawal. Those results support planning for maintenance before deciding whether to stop. [1][2]

## What happened after semaglutide stopped?

In an extension of the STEP 1 trial, participants were followed for a year after semaglutide and the study's lifestyle intervention ended. On average, the semaglutide group regained about two-thirds of its previous weight loss. Some cardiometabolic improvements also moved back toward baseline. [1]

This was an exploratory follow-up of a subset of trial participants. Because both medication and the structured lifestyle intervention stopped, the study describes that combined withdrawal setting rather than every possible maintenance approach. [1]

## What did continued tirzepatide treatment show?

SURMOUNT-4 randomized people who had completed an initial tirzepatide period to either continue treatment or switch to placebo. Continuing treatment maintained and added to the initial weight reduction; switching to placebo led to substantial regain on average. [2]

This was a comparison of continued treatment with withdrawal, not a test of lower-dose or intermittent maintenance. [2]

## How is the long-term decision made?

NIDDK describes the duration of weight-management medication use as depending on benefit and serious side effects. Continued treatment may be appropriate when it supports health and remains tolerable. Cost, other health conditions and individual preferences also belong in medication decisions. [3]

A maintenance conversation can therefore consider several questions: whether the health benefit persists, whether side effects or expense have changed, and how weight and related health measures would be followed if treatment changed. [3]

Maintenance may require ongoing treatment. The plan can be reassessed over time, with a clear understanding of what stopping could mean. [1][2][3]
