Brain fog has been reported by people using finasteride, including in FDA reports involving compounded topical products. Those reports matter, but they do not establish how often it happens or prove that finasteride caused every reported symptom. [1][2]

## What the FDA has reported

In a 2025 alert, the FDA listed brain fog among adverse events reported after use of compounded topical finasteride. Other reported problems included fatigue, insomnia, anxiety and depression. Some reports described symptoms continuing after treatment stopped. [1]

The FDA states that absorption of topical finasteride into the bloodstream is expected. There is no FDA-approved topical finasteride product. Compounded versions are not FDA approved, and the agency has not evaluated their safety, effectiveness or quality before marketing. [1]

The approved oral product's prescribing information lists depression and suicidal thoughts or behavior in its postmarketing section. It explains that voluntary reports come from a population of uncertain size, which limits estimates of frequency and conclusions about causation. [2]

## Why a symptom timeline helps

“Brain fog” may describe trouble concentrating, slower recall, fatigue or feeling mentally unlike yourself. Writing down the specific change gives the person assessing it more to work with than the label alone.

Useful details include when it began, whether it followed a new medicine or formulation, and what changed in sleep, mood or other medications around the same time. Sleep loss, depression and medicines can contribute to fatigue, so timing is a clue rather than a diagnosis. [3]

A discussion about symptoms should include the exact product and whether it is oral or topical. A safety report about one formulation cannot supply a precise personal risk estimate for another. [1][2]

New symptoms deserve a review of their effect on daily life and the treatment's value to you.
