HOMA-IR estimates insulin resistance using fasting glucose and insulin. It describes the relationship between those two measurements rather than directly testing how each tissue responds to insulin. Its value depends on reliable samples and careful interpretation. [1][2]

## What insulin resistance means

Insulin helps glucose move from the bloodstream into cells. With insulin resistance, cells respond less effectively. The pancreas may produce more insulin, keeping blood glucose within the usual range for a time. Insulin and glucose therefore describe related but different parts of the process. [3]

A high fasting insulin result can be relevant, but its meaning depends on the glucose measured with it and the clinical circumstances. [3]

## What HOMA-IR adds

The original homeostasis model assessment used fasting glucose and insulin to estimate the body's resting balance between insulin production and insulin resistance. The original research found correlations with more intensive research measurements, while also reporting limited precision. [1]

The model has been used in clinical research and studies of large populations. It allows researchers to estimate insulin resistance from a fasting blood sample, making it simpler to collect than more intensive measurements. Its developers emphasize that useful results depend on the quality of the inputs and how the estimates are interpreted. [2]

## How this differs from diabetes testing

Prediabetes is usually assessed with blood glucose tests such as A1C, fasting plasma glucose or an oral glucose tolerance test. NIDDK notes that direct insulin-resistance testing is primarily used in research. [4]

HOMA-IR can contribute to a specific assessment, but it does not replace the established tests used to diagnose prediabetes or diabetes. [2][4]

For a report containing HOMA-IR, the useful context is why the result was ordered and how it relates to the glucose results. A provider can explain whether it changes the assessment or the next clinical decision. [2][4]
