Ferritin can help identify low iron stores, one possible explanation for fatigue. Feeling exhausted in the afternoon does not identify the cause by itself. Ferritin is read alongside symptoms and, when appropriate, other blood tests. [1][2]

## Ferritin measures stored iron

Ferritin is a protein that stores iron. A low blood level can point to depleted iron stores. But a high result does not always mean excess iron: infection, inflammation, liver disease and other conditions can raise ferritin. [1]

That is why the same number can mean different things in different circumstances. Recent illness and the other results on the report can change the interpretation. [1][2]

## The other tests answer related questions

A complete blood count, or CBC, includes measures of red blood cells and hemoglobin, the protein that carries oxygen. An iron panel may also include iron in the blood, transferrin, which transports iron, and total iron-binding capacity, or TIBC. These help describe iron transport as well as storage. [2]

A provider can use that combination to investigate whether fatigue is associated with an iron problem. Ferritin alone does not explain why iron stores changed. Heavy menstrual bleeding, pregnancy and problems absorbing nutrients are examples of circumstances that can increase the likelihood of deficiency. [1]

## Does ferritin require fasting?

The collection instructions depend on the tests ordered and the laboratory. MedlinePlus notes that fasting may be requested for ferritin and iron testing. The instructions supplied with the order are the relevant ones, especially when several tests are being collected together. [1][2]

## If iron results do not explain the fatigue

Fatigue can also be associated with sleep disorders, thyroid problems, mood symptoms, medication effects and other conditions. An assessment starts with the pattern of symptoms and health history, which helps determine whether additional testing is useful. [3]
