# New hives in perimenopause: is estrogen involved?

What estrogen and mast-cell research can explain, why hives are not always an allergy, and how symptoms guide testing.

Estrogen can affect cells involved in allergic reactions, but new hives during perimenopause do not establish that hormones caused them. The evidence often cited for this connection includes laboratory experiments, while a person's symptoms still need an assessment of the symptoms and possible triggers. [1][2]

## What the estrogen research found

Mast cells are immune cells involved in allergic responses. In a laboratory study, researchers exposed mast-cell cultures to estradiol, a form of estrogen. They observed release of allergic-response substances and stronger responses when an allergy-triggering pathway was also activated. [1]

The experiment did not test whether perimenopause causes new food allergies, and it cannot diagnose a hormone-driven reaction in someone with itching or hives. [1]

## Hives have several possible explanations

Hives are raised, itchy welts. Short-lived episodes can follow exposure to an allergen, but medicines, heat, exercise and stress can also trigger them. Longer-lasting or recurrent hives are often not caused by an allergy; sometimes no specific cause is identified. [2]

This distinction changes the testing conversation. A recurring rash after a particular exposure poses a different question from welts appearing without a consistent trigger. Allergy testing is most useful when it follows the symptom history and examination. [3]

## What helps clarify the pattern

An assessment can explore when symptoms started, how long episodes last, what happened beforehand, and any new medicines or supplements. Skin tests or specific allergy blood tests may be useful when the history suggests a particular allergen. Broad food-sensitivity testing is not a substitute for that assessment. [3]

A cycle-related pattern is information to describe, alongside the other details.

Hives with throat tightness, trouble breathing, fainting, or tongue or facial swelling can signal a severe allergic reaction. Call 911 for these symptoms. [2]
