# A thinner ponytail: how hair loss is evaluated

Hair-loss patterns, scalp symptoms and recent health changes guide the evaluation. Blood tests are selected when the findings point to a reason.

A thinner ponytail can reflect increased shedding, gradual patterned thinning or more than one cause. An evaluation starts with the pattern and timing of the change, alongside an examination of the scalp. Blood tests are added when the findings suggest a reason for them. [1][2]

Hair changes around midlife deserve the same careful assessment as changes at another age. The timing alone does not establish a hormone problem.

## Timing can reveal a useful clue

Some shedding begins weeks or months after a major physical or emotional stress. Triggers can include a significant illness, surgery, childbirth or a crash diet. This delayed shedding is called telogen effluvium. [1]

That delay is why the history extends beyond the week the hair loss became noticeable. Recent weight changes, changes in eating, new medicines and major illnesses may be relevant even when they seemed unrelated at the time.

Gradual thinning has other possible explanations, including inherited hair loss. Several causes can occur together, so finding one likely contributor may not explain the entire pattern. [1][2]

## The scalp examination matters

A dermatologist looks at the scalp, the distribution of thinning and other areas that may provide clues. They may check how easily hairs come away with a gentle pull. Patchy loss, broken hairs, inflammation and diffuse shedding can point the assessment in different directions. [2]

Describe any itching, pain, redness or scaling, along with changes in the amount of hair shed. Tight hairstyles and other sources of tension also belong in the history. [1]

## Tests follow the suspected cause

Blood tests may be appropriate when the assessment suggests a medical condition, nutritional deficiency or hormone imbalance. A scalp biopsy or other testing may sometimes be needed. Many evaluations can identify a likely cause through the history and examination. [1][2]

The findings determine whether a blood test or another assessment would help.

Sudden, rapid or patchy loss, or hair loss accompanied by scalp pain, itching, redness or scaling, warrants a medical assessment. [1] For a slower change, dated photographs and a short timeline can help describe what you have noticed.
