# Bone density and body composition: which DXA scan are you booking?

A bone-density scan and a whole-body composition scan answer different questions. Check what is measured before comparing results.

A DXA scan can measure bone density or assess body composition, depending on the scan performed. A standard hip-and-spine bone-density exam does not automatically include a whole-body analysis of fat and lean tissue. [1]

Before booking, clarify the question: are you assessing bone health, changes in body composition, or both? The answer determines which measurements you need.

## Bone density is its own measurement

Bone-density DXA measures particular skeletal sites. The International Society for Clinical Densitometry recommends the hip and spine for standard central DXA assessment, with other sites used in specified circumstances. These results help assess bone health and inform fracture-risk decisions. [1]

If the referral is for bone density, a body-fat percentage is not necessarily part of the report. Likewise, a whole-body composition report should not be assumed to replace the dedicated measurements used to assess osteoporosis.

## Body composition separates several parts of body weight

A whole-body DXA analysis reports bone mineral content, fat mass and lean mass. It can also show regional measurements. These provide more detail than the total weight on a bathroom scale. [1]

Lean mass is a tissue measurement, not a direct test of strength. A report describes the measured tissue; how much someone can lift or how easily they move requires a different assessment.

ISCD identifies selected uses for body-composition DXA, including evaluating fat and lean-mass changes in people with obesity undergoing substantial weight loss and assessing people with muscle weakness or poor physical functioning. It also notes uncertainty about the effect of these scans on clinical outcomes. [1]

## Small changes need careful comparison

Differences between reports can reflect scanning conditions as well as changes in the person. ISCD recommends consistent positioning and preparation, including clothing, time of day and recent physical activity. Equipment calibration and measurement precision also matter. [1]

When comparing scans, useful questions are:

- Were both scans the same type of exam?
- Were they performed on the same system, or appropriately cross-calibrated systems?
- Is the measured change large enough to exceed the facility's expected measurement variation?

A clear report should identify what was measured and explain how confidently it can be compared with an earlier result.
