# Feeling wired and tired: what cortisol testing can clarify

Cortisol tests investigate specific hormone disorders. How persistent fatigue and difficulty sleeping are evaluated more broadly.

Feeling tired during the day and alert at night does not identify a cortisol disorder. It describes a pattern that can arise with sleep disruption, stress, caffeine or other health factors.

Cortisol testing has specific clinical uses. It helps investigate disorders in which the body makes too much or too little cortisol, rather than assigning a number to everyday stress. [1]

## The symptom history comes first

Persistent fatigue can be associated with sleep disorders, medicines, iron deficiency, thyroid problems and other conditions. A provider may ask about when it began, changes in daily life, sleep quality and accompanying symptoms, then decide whether examination or testing is needed. [2]

The account can be simple: “I struggle to stay alert at this time, but cannot fall asleep at that time.” Adding when caffeine, medicines or supplements are used helps make the sequence clearer.

Caffeine's effects may last for several hours, and it can contribute to insomnia or anxiety in sensitive people. It can also appear in products other than coffee. [4]

## A cortisol result needs its collection context

Cortisol changes through the day. Depending on the question, testing may use blood, saliva or urine and may involve more than one collection. Medicines, exercise, stress and other circumstances can affect the result. [1]

That is why a result labeled high or low needs the test's timing and purpose. Further evaluation may be needed to establish why it is abnormal. [1]

The presence of fatigue alone does not tell a provider which cortisol test, if any, would be useful.

## “Adrenal fatigue” is a different claim

The Endocrine Society states that “adrenal fatigue” is not an established medical diagnosis and that blood or saliva tests marketed to detect it lack scientific support. The concern is that this label can distract from causes that need a proper assessment. [3]

Actual adrenal insufficiency is a recognized condition. It should not be confused with a wellness explanation that the adrenal glands have simply become tired from a demanding life. [1][3]
