Clarify the medication, formulation, medical rationale and follow-up before comparing GLP-1 offers.

A first GLP-1 treatment discussion should clarify the exact medication being considered, why it may fit the person's health history and how the proposed product will be supplied.

For compounded semaglutide or tirzepatide, there is an additional question: what individual medical need supports using a compounded preparation? FDA says compounded drugs should be used only when an approved drug cannot meet the patient's medical needs. [1][2]

## Identify the finished preparation

Ask for the active ingredient, formulation and route. If another ingredient is included, its identity and reason for inclusion should be explained too.

Compounded medications are not FDA-approved. FDA does not review their safety, effectiveness or quality before marketing, and a compounded drug is not an FDA-approved generic. [1]

## Understand the medical reason

The discussion should connect the proposed treatment to the person's health history, other medicines and the risks relevant to that person. A website's general description cannot complete that assessment.

For routine copies of available drugs, federal compounding restrictions also matter. FDA's April 2026 clarification explains the role of a documented, patient-specific significant difference under 503A. A change such as adding another ingredient does not automatically satisfy that condition. [3]

The explanation should identify the difference that matters for this particular prescription.

## Know who handles the follow-up

The prescriber should explain how questions and new symptoms are reviewed. The dispensing pharmacy should be identifiable and reachable for questions about the preparation, label and handling.

FDA has documented problems involving compounded GLP-1 products, including unclear dosing, measurement errors, fraudulent labeling and inadequate refrigeration during shipping. These findings make product-specific instructions and follow-up important. [2]

Before the conversation ends, it should be clear where to ask a medical question, where to ask a pharmacy question and what information will accompany the order.
