# What “not FDA-approved” means for compounded semaglutide

The distinction applies to the finished preparation. What FDA has reviewed, what it has not, and why the individual medical reason matters.

Compounded semaglutide has not gone through FDA review as a finished drug product. FDA does not review compounded drugs for safety, effectiveness or quality before they are marketed. That is what the “not FDA-approved” disclosure means. [1]

## The ingredient name is only part of the product

A finished medication includes more than its active ingredient. Its formulation, concentration, preparation and labeling affect what a person receives.

FDA has reported concerns with compounded semaglutide that include dosing errors, fraudulent labels and products arriving without adequate refrigeration. It has also warned that semaglutide salt forms are different active ingredients from those used in approved drugs and that the agency is not aware of a lawful basis for their use in compounding. [2]

The prescriber and pharmacist should therefore be discussing the actual preparation, rather than relying on the ingredient name alone.

## Compounding needs a medical basis

FDA says compounded drugs should be used only when a patient's medical needs cannot be met by an FDA-approved drug. Compounding can address a particular need, but it is not the same pathway as approval of a generic medication. [1][2]

The rules also limit routine copies of commercially available drugs. FDA's April 2026 clarification explains that, where a patient-specific significant difference is the basis under 503A, the prescriber must determine and document that difference for the identified patient. Adding another ingredient does not automatically establish it. [3]

## What a clear explanation should cover

For someone offered compounded semaglutide, the discussion should establish why that preparation is being considered, which pharmacy will dispense it and what the exact label will say. It should also explain who answers questions about the prescription and product after delivery.

FDA encourages patients to contact the compounder or telehealth service about unclear instructions or label information. Reports of measurement errors make that communication particularly relevant when a vial or concentration is unfamiliar. [2]
