How to ask a compounding pharmacy about product identity, handling and quality without confusing facility oversight with drug approval.

Useful quality questions name the product and the pharmacy. “Is it high quality?” invites a broad assurance. “Which pharmacy prepares it, and what information will accompany it?” asks for something that can be checked.

Compounded medications are not FDA-approved. FDA does not review their safety, effectiveness or quality before marketing, so identifying the preparation and the responsible pharmacy matters. [1]

## Who prepares and dispenses it?

The service taking an order and the pharmacy supplying the medication may be different organizations. The pharmacy's full name, location and contact details allow its licensure to be checked with the relevant state board.

FDA's online-pharmacy guidance also points to prescription requirements and access to a licensed pharmacist as useful checks. [3]

A response that names only the telehealth platform leaves the preparation question unanswered.

## What exactly will be supplied?

The pharmacy should be able to explain the ingredient, dosage form, concentration and labeling of the dispensed preparation. The prescriber and pharmacist are the appropriate people to clarify how those details relate to the prescription.

This is particularly important when similar-looking product names are used for different formulations. The name of a molecule is not the whole specification of a finished drug.

## What handling information comes with it?

Ask how storage requirements, the relevant use-by information and questions about damaged packaging are communicated. FDA warns that compounded-drug labeling may lack adequate information for safe use and identifies contamination and incorrect active-ingredient amounts as serious quality risks. [2]

Keep the dispensing pharmacy’s contact details with the product instructions.

## What does the facility's status mean?

State-licensed pharmacies and FDA-registered outsourcing facilities have different oversight arrangements. State boards generally oversee the former day to day; registered outsourcing facilities are subject to FDA oversight and current good manufacturing practice requirements. FDA can also inspect state-licensed pharmacies. [1]

If a service cites registration, inspection or accreditation, the useful follow-up is what that claim specifically covers. Facility oversight and approval of the finished medication are separate matters.
